What is the Choices & Changes webinar training?

Choices and Changes, an Institute for Health Care communication designed program presents health care providers with an opportunity to explore their own beliefs about the behavior change process and to test out these beliefs against the current evidence-based best practice.

The Guide for Stroke Recovery (GSR), a stroke self-management resource, accompanies this training to support healthcare providers to empower persons with stroke and their families/caregivers to take an active role in their recovery.  Toronto Stroke Networks (TSNs) and South Riverdale CHC have partnered to offer this professional development training exclusively to staff within the TSNs region. When registering, please include your preferred mailing address as material, required for the day of training, will be sent to you prior to the workshop.

What happens in the training?

In this training, facilitators help you develop a capacity to influence patient health behaviors. By the end of the series participants will be able to:

  • Describe the role of the provider as a facilitator of change;
  • Identify specific strategies to assess the provider’s role in the change process
  • Describe 2 skills to influence changing patient’s behavior, which apply to situations in the clinical setting; and
  • Demonstrate techniques to use in clinical settings to influence change

How will the training be conducted?

  • The training is completed by skilled facilitators who understand the contextual reality of working in health care settings. The training is divided into two sessions which will be conducted via Zoom. Dates for the sessions are:
  • November 05, 2024, 8:30 AM-12:30 PM
  • November 12, 2024, 8:30 AM- 12:30PM

Each participant will receive:

  • Choices and Changes: Clinician Influence and Patient Action Workbook
  • Guide for Stroke Recovery
  • Information and Resources for Persons and Families Recovering from Stroke
  • Certificate of completion (upon attending all 2 webinars)

Note:

  1.  This workshop is interactive and as such attendees will need to have access to a microphone and camera. Participants will also need to sign in from individual devices to allow for breakout rooms” Please ensure to include your preferred mailing address when completing the registration form. Materials and resources will be mailed to you prior to the session. You must have these present during the workshop.
