What is the Online Chronic Disease Self-Management Program?

The Online Chronic Disease Self-Management Program is one of the programs available for individuals living with any long term health conditions. Designed to take place over 6  weeks, the program helps individuals living with long term health conditions learn skills that can help them better manage their symptoms as well as their daily lives.

What happens in the program?

This FREE, six-week, highly interactive workshop takes place entirely online. Participants log on at their own convenience 2-3 times per week for a total of about 2 hours per week.

Who can attend?

The Online Chronic Conditions Self-Management Program (Better Choices, Better Health®), is FREE to individuals across Ontario who are living with one or more chronic conditions such as diabetes, chronic pain, high blood pressure, stroke, cancer, heart or kidney conditions.

Who leads the program?

The workshop is led by two individuals who also live with  long term health conditions and have taken special training.

Who developed the program?

The program was developed at Stanford University and was designed to empower individuals to deal with their illness and improve their quality of life.
