South Riverdale CHC is offering Brief Action Planning (BAP), a Centre for Collaboration, Motivation, and Innovation (CCMI) designed program. BAP is an evidence-based, efficient, and effective tool that helps health care providers support management as well as behavior change that enables patient self-efficacy. BAP focuses on client-centered goal-setting and gives health care providers ways to involve patients in decision making that emphasizes the value of problem-solving, goal setting, creating goal-directed behavior, and following up on these goals with patients in future visits.
The tools and techniques taught in the workshop:
Helps foster a collaborative relationship between clients and their health care provider
Are applicable to any chronic health condition
Can be used in various clinical settings
Are compatible with other self-management techniques
Reinforces language taught in the peer led Stanford program
BAP makes efficient use of health care providers’ time and can be used as part of any routine client interaction with the opportunity to use it over multiple visits.

Members of the health care team, including managers, students, and peer facilitators
How long is this workshop?
BAP includes a series of two webinar sessions delivered using the zoom platform. Each webinar session is 3 hours long. The webinars will be held over 2 consecutive weeks, at the same day and time. Participants must attend both sessions to complete their training.
The sessions will be held on Feb 27, March 06, and practice session on March 13
