Self-Management Toolkit is a modified version of the Chronic Disease Self-Management and Chronic Pain Self-Management programs. It is designed and licensed by the Self-Management Resource Centre and follows a standardized format that has been proven to help people learn to better manage symptoms.
This is a 6-week long phone-based group program for individuals living with long term health conditions. It was developed specifically to reach the most isolated by phone engaging a group of up to 6 people in a 60 minute intervention. The primary audience for these programs are individuals without computer access, or those who are unable to use computer-based technology, or participate in face-to-face programs.
The program will be led by one trained peer facilitator who also live with long term health conditions.
